On Monday, Aban 27, 1392 the coroner of Shohad e Tajrish hospital required Hamid-Reza Moradi imprisoned in ward 350 of Evin prison to sign a medical consent form before surgery.
This medical consent form has reminded to the high risk of the surgery which in mentioned also about common surgery complications, including bleeding embolism, obstruction, infection, shock, foot amputation and even death of the patient.

The imprisoned administrator of Majzooban Noor website, Hamid-Reza Moradi who has heart problems and is suffering from clogged arteries disease which is worrisome ( 70% Artery Blockage) and doctors had warned last year about the risk of his legs amputation if he does not receive immediate medical treatment, was unfortunalely transfer to hospital far too late because of the denial of access to adequate medical care inhumanly by the prison authorities and Judge Salavati so he is now facing with serious risks that involved with his necessary surgery because of the lack of required medical treatment for him. He was supposed to have the surgery on Wednesday 19th Aban 1392 ( Nov 20 2013).
